California Restored Adult Medi-Cal Vision Benefit Jan 1, 2020, But Eyeglasses Coverage Varies by Managed Care Plan
California's adult Medi-Cal vision benefit was restored Jan 1, 2020 via the FY 2019-20 budget (~$17.4M annual for optical services). Adults aged 21+ get comprehensive eye exam every 24 months, frames + single vision/bifocal lenses every 24 months.
Adult coverage was eliminated 2009 (recession-era cut), exam coverage restored July 2010, but eyeglasses for adults remained excluded until 2020 — a 10-year gap during which adults could see an optometrist on Medi-Cal but couldn't get the prescribed glasses. Implementation varies by managed care MCO — Health Net and several others still list materials as restricted to under-21/SNF/pregnant in their provider manuals despite FFS Medi-Cal restoration.
Alameda Alliance moved March Vision → VSP effective Jan 1, 2026. FQHCs must verify coverage per MCO contract.
