Equity Health (formerly South of Market Health Center / SMHC) is a San Francisco FQHC established in 1973 and one of the first community health centers in the city. Became an FQHC in 1989; rebranded to Equity Health on its 50th anniversary in November 2023. Serves 4,600+ patients (18,771 visits CY2023) across four sites in the South of Market, Tenderloin, and Bayview neighborhoods. SFCCC member. Multilingual services covering geriatrics through pediatrics.
