Winters Healthcare is a small grassroots FQHC founded by community members in 2000 and designated in 2005, serving over 3,000 patients in rural Yolo County from a single location in Winters. The organization operates a team-based, relationship-based model of care with integrated primary care, behavioral health, dental, perinatal services, and gender-affirming care, and holds NCQA Patient-Centered Medical Home recognition.
