San Fernando Community Health Center (SFCHC) is an FQHC designated in 2015 serving the San Fernando Valley with comprehensive medical, dental, behavioral health, and OB/GYN services across two locations. It launched a Mobile Medicine Program in fall 2023 to reach unhoused individuals and received $382,000 from DHCS to expand Enhanced Care Management services. Founded 1924. Pivoted to primary care 2013, FQHC status 2015. 284% growth 2016-2019. All bilingual staff.
