CMS Opens $100M MAHA ELEVATE Model for Lifestyle Medicine — FQHCs Eligible
CMS Innovation Center is funding up to 30 cooperative agreements (~$3.3M each over 3 years) for whole-person lifestyle medicine approaches in Original Medicare. FQHCs are explicitly eligible.
This is the first CMS model branded under MAHA priorities. Awards launch October 2026. All proposals must incorporate nutrition or physical activity — signaling policy direction toward 'lifestyle' interventions over traditional SDOH approaches.
