CMS Opens $100M MAHA ELEVATE Model for Lifestyle Medicine — FQHCs Eligible
CMS Innovation Center is funding up to 30 cooperative agreements (~$3.3M each over 3 years) for whole-person lifestyle medicine approaches in Original Medicare. FQHCs are explicitly eligible.
This is the first CMS model branded under MAHA priorities. Awards launch September 2026 (first of two cohorts; second cohort 2027). All proposals must incorporate nutrition or physical activity — signaling policy direction toward 'lifestyle' interventions over traditional SDOH approaches.
