HRSA adds 340B registration questions for covered entities starting October 1
Issue
HRSA said on September 8, 2026 it will update the 340B OPAIS registration, recertification and change-request forms on October 1. Changes include new shipping-address questions that help HRSA decide whether a location should register separately, and Tribal Agreement numbers for FQHC 638 entities.
