HRSA moves health centers to new Form 5A and 5B November 20
Issue
HRSA's Program Assistance Letter 2026-04, dated August 14, 2026, sets Electronic Handbooks scope module changes for November 20, 2026. HRSA will move each center's scope data to new forms, rename enabling services as patient support services and add Other Site and carceral transition site types.
Requests to serve a new underserved population move to a prior approval request.
