This is a clinical-quality SIGNAL derived from public HRSA data — not an official grade. It is peer-relative; centers serving the hardest-to-reach populations naturally post lower raw rates. It is distinct from the employer (Glassdoor) rating.

• Base — real clinical outcome. The national percentile of the center's blood-pressure-control rate (UDS) — a continuous clinical outcome reported by nearly every center.
• Floor — HRSA recognition. The Health Center Quality Leader tier (top 10/20/30% across the FULL clinical measure set) and National Quality Leader domain badges set a floor — so a recognized center is never undervalued by a single measure.
