About the Role
CHW linking SFV community members to health and social services at AAA Comprehensive Healthcare. Conduct community outreach, patient navigation, and social needs screenings for Medi-Cal and uninsured populations across 4 sites.
Requirements
- CA CHW certification preferred (SB 803)
- Bilingual Spanish/English required
- Community outreach or patient navigation experience
- Knowledge of social services in LA County
- Cultural competency with Latino and immigrant communities
