HIV outreach and linkage-to-care for APLA Health — connecting newly diagnosed and out-of-care individuals to HIV treatment, PrEP services, and LGBTQ+-affirming primary care across LA County.
Requirements
- CHW certification or 2+ years HIV outreach experience
- Spanish preferred; other languages (Tagalog, Korean, Mandarin) a plus for LA's diverse LGBTQ+ communities
- LGBTQ+-affirming and non-judgmental approach required
- Valid CA driver's license
