Coordinate care for APLA Health's highest-need ECM enrollees — HIV+ patients, LGBTQ+ individuals with complex needs, and people experiencing housing instability — developing care plans and linking to social services.
Requirements
- RN, SW, or 3+ years care coordination experience
- Experience with HIV-affected or LGBTQ+ populations preferred
- Spanish preferred
- eClinicalWorks preferred
