ECM/Whole Person Care Coordinator in Contra Costa County's FQHC network. Complex Medi-Cal patients — Epic integration across ambulatory, BH, and hospital levels provides full-spectrum care coordination capability.
Requirements
- BA/BS in social work, health administration, or 3+ years complex care coordination
- Epic EHR proficiency preferred
- Spanish language skills preferred
- ECM/Whole Person Care or CalAIM experience strongly preferred
- Knowledge of Contra Costa County social services, housing resources, and Medi-Cal
