ECM/CCM Care Coordinator at CCHS — managing complex Medi-Cal patients across 6 LA sites. Spanish-speaking care coordinators with CalAIM/ECM experience are urgently needed across the organization.
Requirements
- BA/BS in social work, health education, or 3+ years ECM/CCM care coordination
- Spanish fluency required
- NextGen EHR experience a plus
- Valid CA driver's license for community-based visits
- Knowledge of LA County social services, housing, food resources, and Medi-Cal pathways
