Requirements
- BA/BS in social work, health education, or related field; or 3+ years ECM/CCM experience
- Spanish language skills preferred
- OCHIN Epic documentation experience preferred
- Valid CA driver's license for community-based care visits
- Cultural humility with diverse San Gabriel Valley immigrant communities
