Help patients navigate CHCCC services, insurance enrollment, community resources, and social services at the Santa Maria Navigation Center. Connect patients to ECM and Community Supports programs.
Requirements
- Experience in patient navigation, social services, or community outreach
- Bilingual Spanish/English preferred
- Knowledge of Medi-Cal enrollment and community resources
- Strong interpersonal and problem-solving skills
