About the Role
Coordinate ECM care for DAP Health's high-need patients in the eastern Coachella Valley — care plans, social service navigation, and coordination for complex patients including agricultural workers and those living with HIV.
Requirements
- RN, SW, or MA with 2+ years care coordination experience
- Spanish fluency required for Indio/eastern valley population
- eClinicalWorks preferred
- HIV or LGBTQ+ experience a plus
