About the Role
Provide integrated behavioral health in FHN's primary care clinics serving Tulare County's agricultural communities — brief interventions, warm hand-offs, and psychoeducation for depression, anxiety, trauma, and substance use.
Requirements
- LCSW, LMFT, or associate-level with supervision provided
- Spanish fluency required
- Integrated care or FQHC experience preferred
- Agricultural/rural community experience a strong plus
