About the Role
Provide medical nutrition therapy and health education to FHCSD patients managing diabetes, hypertension, and obesity, integrating evidence-based dietary guidance with CalAIM social needs screenings.
Requirements
- Registered Dietitian (RD) or RDN credential required; certified health education specialist (CHES) a plus
- Experience in community health or FQHC population including Medi-Cal patients
- Bilingual Spanish/English strongly preferred
- Familiarity with OCHIN Epic nutrition documentation preferred
