About the Role
Provide comprehensive prenatal case management to high-risk pregnant patients at FHCSD's Logan Heights clinic, coordinating medical, social, and behavioral health services through labor and delivery.
Requirements
- BSW, BSN, or related field; MSW or RN license preferred
- 2+ years case management experience; prenatal or maternal-child health experience a plus
- Bilingual Spanish/English preferred for Logan Heights patient population
- Familiarity with Medi-Cal managed care, CPSP, and WIC programs
