About the Role
Serve as lead ECM case manager for FHCSD's high-need Medi-Cal beneficiaries, delivering intensive care coordination, social needs navigation, and CalAIM program compliance.
Requirements
- Bachelor's in social work, nursing, or related field; MSW preferred
- 2+ years case management in FQHC, hospital, or managed care setting
- CalAIM ECM or Whole Person Care experience strongly preferred
- Bilingual Spanish preferred for San Diego patient population
