Requirements
- High school diploma; CHW experience or certification a plus
- Bilingual Spanish preferred; Tagalog or Somali a plus for other patient populations
- Lived experience in community health, social services, or peer support valued
- Strong interpersonal skills; reliable transportation for community outreach
