About the Role
Provide HIV prevention navigation, linkage to care, and case management support for FHCSD's Ryan White-funded HIV services program, connecting at-risk San Diego residents to testing, PrEP, treatment, and social services.
Requirements
- CHW certification preferred; 1+ year experience in HIV prevention, sexual health, or LGBTQ+ services
- Knowledge of PrEP/PEP access, HIV testing sites, ADAP enrollment, and harm reduction principles
- Bilingual Spanish/English preferred for San Diego's under-resourced Latino communities
- Cultural humility and experience with LGBTQ+, houseless, or substance-using populations
