About the Role
Provide housing-focused care management for FHCSD's CalAIM ECM program, linking unhoused and unstably housed Medi-Cal members to emergency shelter, transitional housing, rental assistance, and community housing resources across San Diego County.
Requirements
- BA/BS in social work, public health, or human services; CHW or case manager certification preferred
- Experience navigating San Diego housing resources: 211, PATH, Alpha Project, SDHC vouchers
- Bilingual Spanish/English preferred; experience with chronically homeless, substance-using, or justice-involved populations
- CalAIM ECM housing documentation and HMIS data entry experience a strong plus
