About the Role
Provide case management and care coordination for FHCSD's women's health program, supporting pregnant and postpartum Medi-Cal patients, survivors of domestic violence, and women with complex social determinants of health needs.
Requirements
- BA/BS in social work, public health, or related field; 2+ years case management in women's health or maternal health
- Experience with CalAIM prenatal ECM, Whole Person Care, or DV support services preferred
- Bilingual Spanish/English preferred; Title X family planning familiarity a plus
- Trauma-informed care approach; experience with SDOH screening and resource navigation
