About the Role
Provide physical therapy services for FHCSD PACE participants, conducting functional mobility assessments, developing individualized therapeutic exercise programs, and delivering fall prevention interventions for frail elderly day center members.
Requirements
- PT with California license required; DPT preferred; geriatric or PACE experience strongly preferred
- Experience with functional mobility assessments, fall risk screening, and Medicare therapy documentation
- PACE IDT participation and care plan documentation experience preferred
- BLS/CPR required; Spanish language skills a plus for bilingual participant communication
