About the Role
Help patients experiencing homelessness navigate housing resources under CalAIM Community Supports. Work alongside ECM care coordinators to connect patients to transitional rent, housing vouchers, and permanent supportive housing in San Diego County.
Requirements
- Experience with HUD/CoC systems, HMIS, or housing navigation
- Knowledge of CalAIM Community Supports and transitional rent
- Bilingual Spanish/English required
- Trauma-informed care and motivational interviewing skills
