CHW at GFHO connecting Fresno's Hmong, Cambodian, and Vietnamese communities to primary care and behavioral health. Hmong-speaking CHWs are critically rare — community trust is built through shared language and lived experience.
Requirements
- CHW certification or 2+ years Southeast Asian community health outreach
- Hmong, Khmer, or Vietnamese language fluency preferred — rare and critically valued
- Lived experience as Southeast Asian refugee or immigrant in Fresno Valley community preferred
- Knowledge of Fresno County social services and Medi-Cal
- Valid CA driver's license
