Requirements
- Medical assistant, CHW, or care coordination background
- Multilingual (Cantonese, Mandarin, or Spanish) required for this population
- Chronic disease care coordination: diabetes, hypertension
- Familiarity with LA County health and social service resources
- FQHC or community clinic experience preferred
