Requirements
- RN, LCSW, or CHW credential with ECM experience
- Bilingual in an Asian language (Cantonese, Mandarin, Vietnamese) strongly preferred
- Chronic disease management: diabetes, hypertension in API population
- NextGen care plan documentation
- SGV community resource knowledge
