Requirements
- BA/BS in social work, health education, or 3+ years ECM/CCM experience
- Spanish fluency required
- Cerner EHR experience preferred
- Valid CA driver's license for community-based visits in SB County
- Knowledge of San Bernardino County social services, housing resources, and Medi-Cal
