Community Health Representative (CHR) at Indian Health Center of Santa Clara Valley, serving AI/AN community members across 13 Santa Clara County sites. Conduct wellness outreach, patient navigation, and cultural support in alignment with tribal traditions and Indian Health Service CHR model.
Requirements
- American Indian or Alaska Native identity strongly preferred
- Community health, social services, or outreach experience
- Knowledge of tribal health concepts and Indian Health Service programs
- Motivational interviewing and health coaching skills
- Valid California driver's license
