About the Role
CHW at LACHC's Skid Row and Downtown LA outreach program. Street-based outreach connecting unhoused individuals to primary care, behavioral health, and permanent supportive housing resources. Faith-based, trauma-informed approach.
Requirements
- CHW certification or 2+ years unhoused/HCH community outreach
- Spanish language skills preferred — significant Spanish-speaking unhoused population
- Lived experience of homelessness or recovery preferred — deeply valued by LACHC
- Comfort with faith-based organizational culture required
- Knowledge of LA Skid Row services, PATH, and Medi-Cal HCH programs
