About the Role
Deliver integrated behavioral health services embedded in La Clinica's primary care teams across East Bay sites, providing brief interventions, warm hand-offs, PHQ-9 screenings, and care coordination for patients with mental health and substance use needs.
Requirements
- Master's degree in social work, counseling, or psychology required
- LCSW, LMFT, LPCC, or associate licensure (ASW, AMFT, APCC) required
- Integrated primary care or co-located behavioral health experience preferred
- Bilingual Spanish/English strongly preferred; OCHIN Epic experience a plus
