Requirements
- BA/BS in social work, health education, or 3+ years ECM/CCM experience
- Spanish fluency required; Punjabi a plus
- NextGen EHR experience preferred
- Valid CA driver's license for community-based visits in Merced County
- Farmworker or agricultural community experience preferred
