About the Role
Provide care coordination for ECM-enrolled patients, connecting them to social services, housing, food, and behavioral health resources. Conduct outreach, home visits, and care plan development in partnership with clinical teams.
Requirements
- Bachelor's degree in social work, public health, or related field preferred
- Bilingual Spanish/English required
- Experience with care coordination or social services
- Knowledge of Marin County community resources
