About the Role
Serve as Enhanced Care Provider at MCC's Larkspur site, managing complex patients enrolled in ECM and CCM programs. Coordinate with care teams, behavioral health, and community resources to address whole-person needs.
Requirements
- Current California NP or PA-C license
- Experience with complex, multi-morbidity patients
- Knowledge of ECM/CCM program requirements preferred
- Strong care coordination and team-based care skills
