Navigate MNHC's HIV-positive patients through care coordination, medication adherence support, Ryan White Part A services, and social determinants screenings in the heart of SF's Mission District.
Requirements
- High school diploma or equivalent; some college or health certification preferred
- Bilingual Spanish/English required; experience working with HIV+ populations or LGBTQ+ communities
- Knowledge of Ryan White Part A services, ADAP, and SF DPH HIV resources
- Experience with Medi-Cal, SSI/SSDI benefit navigation, and housing resources preferred
