About the Role
Serve as the primary community liaison for NEVHC's Enhanced Care Management program, connecting high-need Medi-Cal members to health and social services and conducting home visits across San Fernando Valley.
Requirements
- CHW certification (CHW-CA) required or willing to obtain within 6 months
- Spanish fluency required — bilingual assessment administered at hire
- Valid CA driver's license and reliable vehicle for home visits
- Lived experience in the community strongly preferred
