Coordinate care for NEVHC's highest-need Medi-Cal patients in Complex Care Management and ECM programs — comprehensive assessments, care plans, and coordination across medical, behavioral, and social services.
Requirements
- RN, SW, or MA with 2+ years complex care management experience
- Spanish fluency required
- OCHIN Epic experience preferred
- Case management certification (CCM, ACM) preferred
