About the Role
Provide social work case management and care coordination to complex patients at Open Door's Del Norte County site, connecting clients to housing, mental health, substance use, and social services under CalAIM.
Requirements
- MSW or BSW required; LCSW, ACSW, or ASW licensure preferred
- Case management experience in community health, social services, or behavioral health
- Knowledge of CalAIM Enhanced Care Management and community supports
- Valid California driver's license; willingness to work in rural Del Norte County
