About the Role
Provide pediatric-focused social work case management at Open Door's Eureka site, supporting children and families navigating child welfare, behavioral health, school services, and housing under CalAIM Enhanced Care Management.
Requirements
- MSW required; LCSW, ACSW, or ASW licensure preferred
- Pediatric social work, child welfare, or school-based case management experience strongly preferred
- Knowledge of CalAIM ECM, ACEs, trauma-informed care, and Humboldt County child/family services
- Valid California driver's license; willingness to conduct home visits in rural Humboldt County
