Requirements
- BA/BS in social work, health education, or 3+ years ECM/CCM experience
- Spanish fluency required
- Epic EHR experience preferred
- Valid CA driver's license for community-based visits in the Coachella Valley
- Knowledge of Riverside County social services, Medi-Cal, and CalAIM pathways
