CHW outreach at Salud Para La Gente — connecting Santa Cruz County farmworkers to primary care, Medi-Cal enrollment, and social services in the fields and communities of the Pajaro Valley.
Requirements
- CHW certification or 2+ years outreach in agricultural communities
- Spanish fluency required; Mixtec or Triqui a strong plus
- Valid CA driver's license — field travel required
- Farmworker community lived experience highly valued
