CHW at SFCHC connecting SF's LGBTQ+ and AAPI communities to primary care and ECM. Tenderloin, Chinatown, and SoMa outreach — LGBTQ+ affirming and multilingual Chinese/Vietnamese community navigation.
Requirements
- CHW certification or 2+ years LGBTQ+ or AAPI community outreach
- Cantonese or Vietnamese language skills preferred
- LGBTQ+-affirming approach required — personal identity preferred
- Knowledge of SF AAPI and LGBTQ+ community resources and Medi-Cal
- SF Tenderloin/SoMa/Chinatown community lived experience strongly valued
