About the Role
CHW for SOS's health mobile RV program — bringing primary care to OC's encampments, shelters, and transitional housing sites. Bilingual Spanish/English CHW with HCH outreach experience urgently needed.
Requirements
- CHW certification or 2+ years HCH/unhoused outreach experience
- Spanish fluency required — significant Spanish-speaking unhoused population in OC
- Valid CA driver's license for mobile RV travel across Orange County
- Trauma-informed, harm-reduction approach required
- Knowledge of OC social services, PATH, and Medi-Cal HCH programs
