About the Role
Manage a caseload of high-utilizer Medi-Cal members enrolled in St. John's Enhanced Care Management program, conducting home visits, coordinating specialty referrals, and addressing social determinants of health for South LA's under-resourced communities.
Requirements
- BA/BS in social work, public health, or related field; CHW or care manager certification preferred
- Bilingual Spanish/English required for South Central LA patient population
- Knowledge of CalAIM ECM documentation requirements and Medi-Cal managed care plans
- Experience with SDOH screening, community resource navigation, and motivational interviewing
