About the Role
Conduct community outreach and patient navigation for St. John's South LA clinics, connecting uninsured and high-need Medi-Cal members to healthcare, social services, housing resources, and community supports.
Requirements
- CHW certification (California) required or in progress; 1+ year community outreach experience
- Bilingual Spanish/English required; lived experience in South LA communities strongly valued
- Knowledge of Medi-Cal enrollment, sliding fee scale, and community resource landscape
- Valid driver's license; comfortable with field-based community outreach work
