About the Role
Provide Targeted Case Management for St. John's Medi-Cal members with serious mental illness, developmental disabilities, or chronic conditions, linking patients to community services and coordinating care plan implementation in South LA.
Requirements
- BA/BS in social work, psychology, or related field; TCM or case manager certification a plus
- Bilingual Spanish/English required; experience with LA County DHS or DCFS systems preferred
- Knowledge of Medi-Cal TCM documentation standards and DHCS compliance requirements
- Experience with seriously mentally ill, justice-involved, or homeless populations preferred
