Requirements
- BA/BS in social work, health education, or 3+ years ECM/CCM experience
- Spanish language skills preferred
- NextGen EHR experience preferred
- Valid CA driver's license for community-based ECM visits
- Knowledge of Sacramento County social services and Medi-Cal
