About the Role
Provide Ryan White-funded medical case management for San Ysidro Health's HIV-positive border community patients, coordinating ADAP enrollment, adherence support, and social services linkage for a predominantly binational, Spanish-speaking population.
Requirements
- BA/BS in social work or health-related field; Ryan White case management experience required
- Bilingual Spanish/English required; experience with binational (US–Mexico) patient navigation preferred
- Knowledge of ADAP, HIV care continuum, PrEP access, and Medi-Cal Ryan White coordination
- Cultural competence with LGBTQ+, undocumented, and border-crossing patient communities
